Job summary

Bristol Hospice in Madison, WI is seeking a Hospice Social Worker to support patients and families facing terminal illness. The role emphasizes compassionate psychosocial care, crisis intervention, and collaboration with the interdisciplinary team to align care with patient values.

Applicants should hold a Master’s in Social Work, have at least two years of supervised health care experience (hospice preferred), and be willing to travel within the Bristol Hospice service area.

Qualifications

• Master’s degree in social work from an accredited university.
• Minimum two years of supervised experience in health care; hospice experience preferred.
• Must understand hospice philosophy and death/dying issues; willing to travel.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the nurse and team.
• Provide social evaluations and plan interventions based on evaluation findings.
• Counsel patients and families; provide crisis intervention when necessary.
• Educate on cultural needs, advance directives, and interpersonal dynamics.
• Maintain clinical records and participate in interdisciplinary group meetings.
• Assist with funeral planning, finances, and health care decisions.
