Blue Ridge Care in Virginia is seeking a Hospice Social Worker to support patients, families, and caregivers, addressing emotional and psychosocial needs within end-of-life care, collaborating with the interdisciplinary team to plan care.

You will provide counseling, assessments, develop interventions, document in EMR, coordinate resources, and travel throughout the service area in Loudoun County.

Formación

• Master of Social Work required.
• At least one year of experience in a health-care setting; hospice experience preferred.
• Understanding of hospice philosophy and end-of-life issues.
• Strong communication, documentation, and organizational skills.
• Ability to travel throughout the Blue Ridge Care service area in Loudoun County.

Responsabilidades

• Assess psychosocial status of patients and caregivers within terminal illness.
• Assess home safety and environment; propose alternative plans as needed.
• Conduct social evaluations and develop coping interventions.
• Provide counseling to patients and families on stress and end-of-life issues.
• Maintain accurate clinical records and document in the EMR at the bedside.
• Offer guidance on community resources and referrals; connect with agencies.
• Collaborate as liaison between families, hospice team, and partners.
• Participate in IDT meetings and contribute to care planning.
• Engage in quality improvement activities.
