Qualifications

• Graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education.

Responsibilities

• Assesses psychosocial status of patients and families related to terminal illness.
• Educates on advanced directives and assists families in crisis intervention.
• Maintains clinical records and collaborates with community agencies.
