Job summary

Luminary Hospice is seeking a PRN Master’s or Bachelor’s level Social Worker to implement standards of care for medical social work services. The role requires MSW or CSWE-accredited BSWS, CPR certification, and a valid driver’s license. The social worker will assess patients and families, educate the care team on cultural needs, and maintain records.

Qualifications

• MSW or CSWE-accredited BSWS degree required
• Strong verbal and written communication skills
• Ability to educate interdisciplinary groups and provide crisis intervention
• Cultural sensitivity and family-centered care

Responsibilities

• Assesses psychosocial status of patients and families/caregivers related to terminal illness and communicates findings to the care team
• Conducts social evaluations including family dynamics and caregiver abilities, and plans interventions
• Educates interdisciplinary groups on patient/family cultural needs and traditions
• Maintains clinical records for all social work referrals
• Assists in preparation of advanced directives and provides information/referral services
• Serves as liaison between patients/families and community agencies
• Maintains collaborative relationships with hospital/organization personnel to support patient care
• Participates in development of the plan of care with patient and family
• Assists in understanding social/emotional factors related to health problems and end-of-life issues
• Participates in quality assessment and performance improvement activities
• Assists in planning funeral arrangements and related decisions
• Other duties as delegated by Clinical Supervisor
