Bristol Hospice in Walnut Creek, CA is seeking a Hospice Social Worker to join our patient- and family-centered team. The role supports end-of-life care, psychosocial assessment, counseling, and collaboration with the interdisciplinary team in a day shift.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Undergraduate B.A. in Social Work is acceptable; pay based on degree type.

Responsibilities

• Assess the psychosocial status of patients and families related to terminal illness and communicate findings to the care team.
• Provide psychosocial assessments at the patient’s residence as needed and plan interventions.
• Counsel patients and families, provide crisis intervention, and assist with advance directives.
