Job summary

Bristol Hospice in Redding, CA is seeking a Hospice Social Worker to assess and support patients and families facing terminal illness. The role involves coordinated care with the interdisciplinary team and education on care planning and advanced directives.

The position requires a Master’s in Social Work, at least two years in healthcare with hospice experience preferred, and a willingness to travel within the Bristol Hospice service area.

Qualifications

• Master's degree in social work from an accredited university.
• Minimum of two years of documented supervised experience in health care; hospice experience preferred.
• Certified Hospice and Palliative Social Worker desirable; understand hospice philosophy and death/dying issues.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers and communicate findings to the nurse and interdisciplinary team.
• Provide social work assessments at the patient’s residence and plan follow-up when safety or access is an issue.
• Carry out social evaluations including family dynamics, caregiver abilities, and risk factors; plan interventions accordingly.
• Counsel patients and families to address stress and coping; provide crisis intervention when necessary.
• Educate the team on culturally appropriate needs and preferences of patients and families.
