Job summary

VIA Health Partners in Charlotte, NC is seeking a licensed hospice social worker to join our interdisciplinary team providing compassionate care at the end of life. You will assess patients and families, develop and adjust plans of care, and advocate for needed resources.

You will document assessments and interventions, participate in family meetings, and collaborate across departments to coordinate services and transitions to grief care when appropriate.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) required.
• Computer proficiency is required.
• Medical social work or hospice experience preferred.

Responsibilities

• Conduct psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develops, implements, and evaluates care plans addressing counseling and education needs while respecting patient/family dignity.
• Document assessments, care plans, and interventions per guidelines.
• Participate in interdisciplinary team meetings to coordinate patient care goals.
• Provide education on hospice philosophy, advance care planning, and end-of-life issues.
• Conduct family meetings and coordinate referrals to grief services when appropriate.
