Job summary

VIA Health Partners is seeking a licensed clinical social worker to deliver comprehensive psychosocial assessments and care planning for patients and families in a hospice setting. The role emphasizes collaboration with interdisciplinary teams and adherence to VIA documentation standards.

The position requires LCSWA/LCSW licensure in NC and SC, a CSWE-accredited MSW, and demonstrated experience in medical social work within hospice. On-site work with a supportive leadership structure is provided.

Qualifications

• Bachelor’s degree required for BSW role.
• Master’s degree in social work from CSWE-accredited institution preferred.
• Licensed as LCSWA or LCSW in NC/SC per state boards.

Responsibilities

• Conducts psychosocial assessments of patients, families, and caregivers.
• Develops, implements, and evaluates plans of care.
• Collaborates with interdisciplinary teams to provide culturally sensitive care.
• Provides education on hospice philosophy and advance care planning.
• Maintains clinical documentation per VIA guidelines.
• Manages time, caseload, and referrals for resources and support.
