Job summary

Affinity Hospice Holdings, LLC is seeking a Licensed Master Social Worker (LMSW) to assess psychosocial symptoms and develop objectives, provide psychoeducation, supportive counseling, and casework to patients and families.

The LMSW will collaborate with the Interdisciplinary Group (IDG), participate in on-call rotations, and support bereavement services while adhering to hospice regulations. This is a full-time role with comprehensive benefits.

Qualifications

• MSW degree required and licensed LMSW in the state of employment.
• Clinical experience or formal training in death, dying and bereavement.
• Knowledge of hospice regulations (state and federal).
• Ability to work flexible schedule, including some evenings and weekends.
• Valid driver's license and auto liability insurance.

Responsibilities

• Provide psychosocial services to terminally ill patients and their families.
• Regularly assess the patient and family psychosocial needs.
• Deliver psychoeducation, supportive counseling, and casework interventions.
• Consult with the Interdisciplinary Group (IDG) during weekly meetings.
• Assist family with funeral planning, finances, and healthcare decisions.
• Participate in on-call rotation with the social work team.
• Support department goals and initiatives.
