Job summary

Preferred Hospice in Missouri is seeking an experienced Social Worker (MSW preferred) to join our hospice interdisciplinary team serving the Farmington, MO area. The ideal candidate will have medical care exposure and reside within a commutable distance to the Farmington office.

Responsibilities include conducting psychosocial assessments, coordinating with community resources, assisting with admission paperwork, and supporting patients and families through grief.

Qualifications

• Master's in Social Work preferred; CSWE-accredited program.
• Bachelor's in Social Work considered.
• Licensed social worker (LSW/LCSW) preferred or eligible.
• Experience in hospice or medical settings is a plus.

Responsibilities

• Visit patients at home, SNF, ALF, or hospital.
• Complete psychosocial assessments and document in EMR.
• Coordinate with community resources for in-home services.

Preferred Hospice is a well-established hospice company with 5 offices across the state of Missouri---Dexter, Farmington, St Louis, Columbia, Ozark.

We are looking for an experienced Social Worker (MSW preferred) to work as part of our hospice interdisciplinary team serving the Farmington, MO area. The ideal candidate would have some medical care experience and live within a commutable distance to the office.

JOB DUTIES:

• The Hospice Social Worker visits wherever the patient resides---private home, skilled nursing facility, assisted living facility, hospital.
• The Hospice Social Worker completes a psychosocial assessment on each assigned patient in a timely manner, and documents in the electronic medical record.
• The Hospice Social Worker utilizes community resources for patients, such as in-home services, Meals on Wheels, etc.
• The Hospice Social Worker assists the patient, family and caregiver in coping with issues related to death and dying and participates in grief support as needed.
• The Hospice Social Worker provides information, completes hospice admission paperwork and educates patients, families and the community on all aspects of hospice services.

QUALIFICATIONS:

• The successful candidate will have graduated from a School of Social Work approved by the Council of Social Work Education with a Master's Degree (preferred). Bachelor's Degree in Social Work candidates will be considered. (***Other degrees/areas of study do not meet state guidelines and can not be considered.***)
• Work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time. Flexibility with scheduling and hours a must.
• Hourly rate, mileage reimbursement provided. Compensation is based on years of experience.
• Computer skills for charting/EMR, basic use of Word, Excel, Email; smart phone for mileage apps, etc.
• Willing to train the right person. A loving compassionate heart is a must.
