Job summary

St. Croix Hospice in Oak Creek, WI is seeking a compassionate Social Worker to join our patient care team. You will meet with patients and families, collaborate with the clinical team, and connect families with community resources in alignment with each Plan of Care.

Qualifications

• Bachelor’s degree in social work from an accredited program.

• Collaborate with the interdisciplinary team, maintain HIPAA compliance.

At St. Croix Hospice, the Social Worker is an essential member of the patient care team, providing compassionate emotional and practical support to patients and their families in alignment with each individual’s Plan of Care. In this role, the Social Worker meets with patients and their loved ones, collaborates closely with the clinical team, and connects families with appropriate community resources and services.

QualificationsSocial Worker Requirements & Core Responsibilities:

• Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
• Current Social Work licensed in the state(s) of practice
• Five (5) licensure levels:
• Certified Social Worker (CSW)
• Certified Advanced Practice Social Worker (CAPSW)
• Certified Independent Social Worker (CISW)
• Licensed Clinical Social Worker (LCSW)
• Social Work Training Certificate (SWTC)
• Ability to pass background, consumer report, and drug test.
