Job summary

RCM Healthcare Services partners with a nonprofit hospice program in New York City to recruit a Hospice Social Worker (LCSW). The role provides emotional support, case management, and community resource coordination in a field-based capacity across NYC.

You will collaborate with the IDT, conduct biopsychosocial assessments, develop individualized plans of care, and help patients access essential services to improve quality of life.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Active NYS LCSW and NYS Registration.
• Valid driver’s license and access to a vehicle.
• Minimum 3 years of clinical experience in a healthcare setting.
• Hospice/Palliative care or related field experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Serve as the social work representative on the Hospice & Palliative Care IDT.
• Complete biopsychosocial/spiritual assessments within required timeframes.
• Provide supportive counseling, crisis intervention, and behavioral strategies.
• Develop social work plans of care with goals, interventions, and referrals.
• Document timely and accurately in the EMR.

Hospice Social Worker (LCSW) Compensation and Benefits

Salary: $68,838 – $80,000
$15,000 Sign-on Bonus

Location

• Regional Coverage: Queens, NY
• Field based position (inpatient, home care or nursing home setting)
• Car and driver license required

Position Summary

We are partnering with a leading Hospice & Palliative Care program, a highly respected nonprofit organization, to recruit a Hospice Social Workers (LCSW) to join their growing Palliative Care & Hospice teams across NYC. As a key member of the Interdisciplinary Team (IDT), the Hospice Social Worker provides emotional support, counseling, case management, and community resource coordination to patients and families in a field-based capacity. You will conduct biopsychosocial assessments, develop individualized plans of care, and help patients access critical services that support quality of life. • Ability to manage complex cases, navigate community resources, and support families during the dying process
