We are partnering with a leading Hospice & Palliative Care program, a highly respected nonprofit organization, to recruit a Hospice Social Workers (LCSW) to join their growing Palliative Care & Hospice teams across NYC. As a key member of the Interdisciplinary Team (IDT), the Hospice Social Worker provides emotional support, counseling, case management, and community resource coordination to patients and families in a field-based capacity. You will conduct biopsychosocial assessments, develop individualized plans of care, and help patients access critical services that support quality of life.
