Job Details

• Provide psychosocial support and social work services to hospice patients and their families in various settings, including inpatient, home care, and nursing homes.
• Serve as a member of the hospice interdisciplinary team (IDG) and act as a liaison between hospice staff and inpatient/nursing home staff.
• Develop personalized social work plans of care and goals tailored to patients' needs.
• Collaborate on discharge plans for patients admitted to inpatient facilities.
• Support patients and families through the pre- and post-bereavement phases of hospice care.
• Maintain accurate clinical records and contribute to care coordination.
• Advocate for patients and provide resources to support coping and quality of life.
  
  

Skills You Need

• Master's degree in Social Work (MSW) from an accredited school.
• Valid New York State MSW licensure (LMSW or LCSW) and a NYS driver's license.
• Minimum of one year of social work experience in a healthcare setting.
• Strong organizational, communication, and interpersonal skills with the ability to work in an interdisciplinary team environment.
