Job summary

Regional Hospice is seeking an experienced Social Worker to join our interdisciplinary team serving the Lebanon, MO area. The role involves conducting psychosocial assessments, coordinating with families, and connecting patients with community resources.

MSW preferred; Bachelor’s degree in Social Work will be considered. Normal hours are Monday through Friday, 8am to 5pm, with on‑call coverage as needed. Travel to patient residences is required and mileage is reimbursed.

Qualifications

• MSW preferred; Bachelor's degree in Social Work considered if meet state guidelines.
• Hours: Mon-Fri, 8am-5pm with crisis on-call; scheduling flexibility required.
• Hourly rate with mileage reimbursement; compensation based on experience.
• Proficient in charting/EMR, Word, Excel, Email; smartphone for mileage apps.
• Willing to train; compassionate heart required.
• Bereavement knowledge a strong plus.

Responsibilities

• Visit patients at residences—home, SNF, ALF, or hospital.
• Complete psychosocial assessments and document in EMR.
• Utilize community resources for in-home services and support.
• Assist patient, family and caregiver with death and dying issues; provide grief support as needed.
• Provide information, complete hospice admission paperwork, educate on hospice services.

Regional Hospice is a well-established hospice company with 3 offices covering the southwest corner of Missouri---Lebanon, Springfield, and Joplin

We are looking for an experienced Social Worker (MSW preferred) to work as part of our hospice interdisciplinary team serving the Lebanon, MO area. The ideal candidate would have some medical care experience and live within a commutable distance to the office.

JOB DUTIES:

• The Hospice Social Worker visits wherever the patient resides---private home, skilled nursing facility, assisted living facility, hospital.

• The Hospice Social Worker completes a psychosocial assessment on each assigned patient in a timely manner, and documents in the electronic medical record.

• The Hospice Social Worker utilizes community resources for patients, such as in-home services, Meals on Wheels, etc.

• The Hospice Social Worker assists the patient, family and caregiver in coping with issues related to death and dying and participates in grief support as needed.

• The Hospice Social Worker provides information, completes hospice admission paperwork and educates patients, families and the community on all aspects of hospice services.

QUALIFICATIONS:

• The successful candidate will have graduated from a School of Social Work approved by the Council of Social Work Education with a Master's Degree (preferred). Bachelor's Degree in Social Work candidates will be considered. (***Other degrees/areas of study do not meet state guidelines and will not be considered.***)

• Work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time. Flexibility with scheduling and hours a must.

• Hourly rate, mileage reimbursement provided. Compensation is based on years of experience. Recent company-wide increase in mileage.

• Computer skills for charting/EMR, basic use of Word, Excel, Email; smart phone for mileage apps, etc.

• Willing to train the right person. A loving compassionate heart is a must.

• Some knowledge of bereavement a strong plus.
