Job summary

Three Oaks Hospice in the San Antonio area is seeking a licensed social worker to provide coordinated in-home psychosocial support to patients across all ages, conducting assessments, counseling, and referrals. You will collaborate with the IDG/IDT, promote comfort, dignity, and family support, and help navigate community resources and advance directives.

Required LMSW license, state certification, knowledge of local resources, and reliable transportation.

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• If has a bachelor’s degree and is hired after 12/02/2008, must be supervised by a Master of Social Work
• Current and valid license as a LMSW
• Working knowledge of community resources. Good interpersonal skills. Hepatitis profile.
• Current State Certification
• Reliable transportation. Valid Driver’s License and current auto liability insurance

Responsibilities

• Provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments, counseling, and referrals.
• Assesses and completes assessments within five days of hospice care election to identify patient and family needs.
• Assist with advance directives, family counseling and discharge planning as indicated.
• Participate in IDG/IDT meetings and coordinate patient care with the team.
• Maintain patient privacy and participate in QAPI activities.

Position Summary

Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family.

Essential Duties

• Assesses and completes the Comprehensive Assessment within five days of the patient’s election of hospice care to identify patient and family’s psychosocial, financial, environmental and community needs as evidence by POC documentation, clinical records, IDG/IDT meetings and community resource referrals.
• Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.
• Uses effective interpersonal communication skills, both verbal and written.
• Meets mandatory continuing education requirements of the Hospice and licensing board.
• Demonstrates commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
• Provides patient and family/care giver counseling around issues of end-of-life, death, and grief.
• Promote hospice philosophy and administrative policies to ensure quality of care.
• Attends IDG/IDT meetings, participate in the patient care planning process and coordinating and collaborating with the

• Accepts clinical assignments that are consistent with education and competence to meet the needs of the patients.
