Responsibilities
- Assures that social service activities are integrated and coordinated within the hospice.
- Develops specific, measurable, realistic plans and objectives with the goal of quality of life and safety in mind.
- Performs psychosocial and bereavement assessments on all patients and families.
- Serves as a resource to patients and families for questions and emotional support. Identifies and addresses psychosocial needs of patients and families.
- Demonstrates knowledge of clinical social work.
- Communicates and collaborates appropriately and clearly to all members of the hospice and facility/referral source teams.
- Facilitates admission meetings and demonstrates competency and proficiency in explaining Medicare hospice regulations, advanced care planning, and hospice education for patients, families, and referral sources.
- Demonstrates the ability to be flexible, organized and function under stressful situations.
- Makes referrals to outside agencies or groups as needed.
- Demonstrates cultural competency in work.
- Documents in accordance with Medicare regulations and hospice department standards.
- Is an active member of the team meetings.
- Participates in the on‑call supportive services rotation.
- Participates in committees and projects as appropriate.
- Carry out QAPI roles and responsibilities as assigned.
Qualifications
- Graduation from a four‑year college or university with a degree in social work.
- MSW required in accordance with Medicare Conditions of Participation.
- Personal computer experience required.
- Hospice or Medical Social Work experience preferred.
- Maintains a current PA driver's license and insurance on personal vehicle.
