Job summary

Faith & Family Hospice in Massachusetts is seeking a compassionate Full-Time Hospice Social Worker (MSW) to support patients and families at the end of life. You will provide counseling, advocacy, and resource coordination as part of our small, interdisciplinary team.

You will conduct psychosocial assessments within five days of admission, assist with advance care planning, connect families with community resources, and document in accordance with hospice standards.

Qualifications

  • Master's Degree in Social Work from a CSWE accredited program.
  • Licensed as a Social Worker through the Massachusetts Board of Social Worker.
  • Three years recent experience in acute, long-term care or home care/hospice, with hospice preferred.

Full-Time Hospice Social Worker (MSW)

We are seeking a compassionate, skilled, and mission-driven Social Worker (MSW) to join our small and dedicated hospice team. About the Role

As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.

Key Responsibilities

  • Conduct in-person psychosocial assessments of patients and families within 5 days of admission to hospice per Medicare guidelines
  • Provide emotional support, counseling, and crisis intervention
  • Ability to self-organize, schedule and triage visits based on clinical need
  • Assist patients and caregivers with advance care planning and end-of-life decision-making
  • Connect families with community resources and supportive services
  • Facilitate discussions regarding patient goals, family dynamics, coping, and bereavement needs
  • Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team
  • Advocate for patient and family needs while promoting dignity, comfort, and quality of life
  • Complete timely and accurate documentation in accordance with hospice standards

Reporting Relationship: Executive Director

Qualifications: Master's Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
