What You'll Do as a Hospice Social Worker

As a Hospice Social Worker, you will carry out our Mission of Love by assisting patients, families and caregivers with emotional and practical issues at the end of life with the goal of enhancing quality of life.

Our Hospice Social Worker is responsible for the following:

• Assuring that social service activities are integrated and coordinated within the hospice.
• Developing specific, measurable, realistic plans and objectives with the goal of quality of life and safety in mind.
• Performing psychosocial and bereavement assessments on all patients and families.
• Serving as a resource to patients and families for questions and emotional support, identifying and addressing psychosocial needs.
• Demonstrating knowledge of clinical social work.
• Communicating and collaborating appropriately and clearly to all members of the hospice and facility/referral source teams.
• Facilitating admission meetings and demonstrating competency and proficiency in explaining Medicare hospice regulations, advanced care planning, and hospice education for patients, families, and referral sources.
• Demonstrating the ability to be flexible, organized and function under stressful situations.
• Making referrals to outside agencies or groups as needed.
• Demonstrating cultural competency in work.
• Documenting in accordance with Medicare regulations and hospice department standards.
• Being an active member of the team meetings.
• Participating in the on‑call supportive services rotation.
• Participating in committees and projects as appropriate.
• Carrying out QAPI roles and responsibilities as assigned.

What You Need as a Hospice Social Worker

• Graduation from a four‑year college or university with a degree in social work.
• MSW required in accordance with Medicare Conditions of Participation.
• Personal computer experience required.
• Hospice or Medical Social Work experience preferred.
• Maintains a current PA driver's license and insurance on personal vehicle.
