As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.
Reporting Relationship: Executive Director
Master's Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
