Qualifications

• Master's Degree in Social Work (CSWE accredited).
• Licensed Social Worker in Massachusetts.
• 3 years of recent experience in acute, long-term care, or home care/hospice; hospice/homecare preferred.
• Knowledge of Medicare/Medicaid, financial aid, and local social services.

Responsibilities

• Conduct psychosocial assessments within 5 days of admission per Medicare guidelines.
• Provide counseling, emotional support, and crisis intervention.
• Self-organize, schedule, and triage visits by clinical need.
• Assist with advance care planning and end-of-life decision-making.
• Connect families with community resources and supportive services.
• Lead discussions on goals, family dynamics, coping, and bereavement needs.

About The Role

As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.

Key Responsibilities

• Conduct in-person psychosocial assessments of patients and families within 5 days of admission to hospice per Medicare guidelines

• Assist patients and caregivers with advance care planning and end-of-life decision-making

• Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team
• Advocate for patient and family needs while promoting dignity, comfort, and quality of life
• Complete timely and accurate documentation in accordance with hospice standards

Qualifications

Master's Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
