Job summary

The Care Team Hospice is seeking a licensed Medical Social Worker (MSW) to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and families. The role collaborates with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Responsibilities include psychosocial assessments, care planning, team participation, and accurate EMR documentation using HCHB. On-call coverage may be required with additional compensation.

Qualifications

• Master's in Social Work required.
• Experience in hospice or end-of-life care preferred.
• Strong psychosocial assessment and counseling skills.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling and emotional support to patients and families.
• Assist with advanced care planning and coordination of community resources.
• Participate in interdisciplinary team meetings.
• Document services accurately in the EMR HCHB.
