Health Watch Health Care, Home Health, Hospice and Affiliates seeks a Medical Social Worker to implement standards of care for social work services in hospice. The role involves psychosocial assessments, family counseling, and coordination with the interdisciplinary team within the home/hospice setting.

The position requires an MSW or related social work degree with supervision, plus experience in healthcare; strong communication and organizational skills are essential.

Qualifications

• Master of Social Work (MSW) degree from an CSWE-accredited program, or a bachelor’s in social work with MSW supervision as described in related regs.
• One year of social work experience in a health care setting.
• Understanding hospice philosophy and death/dying issues.

Responsibilities

• Assesses psychosocial status of patients and families/caregivers and communicates findings to the care team.
• Carries out social evaluations and plans interventions based on evaluations.
• Provides information and referral services for patients and families.
• Maintains clinical records for referred patients.
• Assists in discharge planning and securing durable power of attorney and funeral arrangements as needed.
• Participates in development of plan of care and interdisciplinary meetings.
