Job summary

HOMECARE & HOSPICE in Allegany, NY seeks a Social Work Manager to lead social services for patients and families in the home care and hospice setting. Under the Chief Nursing Officer, you will supervise staff, develop care plans with RNs and physicians, and ensure timely documentation and compliance.

The role emphasizes counseling, crisis intervention, liaison with community agencies, bereavement support, and participation in interdisciplinary teams.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Current license to practice social work in New York.
• One year of social work experience in a health care setting.
• Knowledge of federal/state hospice/home care regulations preferred.
• Demonstrated organizational and supervisory skills.
• Effective oral and written communication abilities.
• Acceptable driver's license with current insurance.

Responsibilities

• Supervises, evaluates, guides, and maximizes staff effectiveness.
• Establishes initial care plans with RN and attending physician.
• Performs, reviews, and approves social work assessments.
• Documents clinical records timely with patient/family visits.
• Provides counseling and referrals for psychosocial needs.
• Attends interdisciplinary team and quality meetings as required.
• Coordinates social work services with community agencies.
• Manages bereavement plan of care and related reports.
• Ensures supervision and family independence services are delivered.
• Prepares required reports and participates in staff education.
