Job summary
Ascend Hospice & Palliative Care PA is actively seeking a Registered Nurse for on-site weekend PRN coverage in North Wales, PA. The RN will assist with on-call, case management, and visits to support continuous, coordinated hospice care for patients and families.
The position requires an Associate's Degree in Nursing, an active state RN license, and at least 2 years of nursing experience (hospice preferred). Must have a valid driver's license and be willing to travel.
Qualifications
- Active State RN license
- 2 years' nursing experience, hospice preferred
- Valid driver's license and reliable transportation
- Willingness to travel
- Strong verbal and written communication skills
Responsibilities
- Perform initial patient and family assessments to determine hospice care needs
- Regularly reassess nursing needs and adjust care plans
- Utilize assessment data to determine care plan requirements
- Coordinate with healthcare team to ensure continuous, well-coordinated care
- Counsel patients and families on end-of-life needs
- Initiate appropriate nursing procedures focused on palliative care
- Communicate with physicians about patient needs and condition changes
- Supervise Hospice Aides as assigned
