We’re seeking a dedicated Registered Nurse to serve as an on‑call expert for our growing patient base. Key Responsibilities

• Respond promptly to after‑hours calls for symptom management, crisis care, or family support.
• Provide in‑home skilled nursing care, assessments, and interventions.
• Deliver emotional guidance to patients and caregivers during sensitive moments.
• Collaborate with physicians and interdisciplinary team to adjust care plans as needed.
• Document all visits and phone calls accurately and timely in our EHR system.
• Educate families on comfort care and end‑of‑life expectations.
• Triage patient needs via phone and determine appropriate next steps.

Qualifications

• Active RN license in Minnesota.
• Hospice, oncology, or palliative care experience required.
• Strong clinical judgment and ability to remain calm in urgent situations.
• Excellent communication skills with patients, families, and medical teams.
• Willingness to work evenings, overnights, weekends, and holidays on a rotating schedule.
• Reliable transportation and valid driver’s license.
