Job summary

UnitedHealth Group is hiring a Hospice RN Case Manager in Danville, PA. You will take full nursing responsibility for delivering the Plan of Care for each hospice patient, collaborating with professionals to implement treatment strategies that promote overall well-being.

The role requires an RN licensure in the state of practice and at least 1 year of clinical experience. The position offers a full-time schedule with a comprehensive benefits package and opportunities for incentive programs.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience preferred.
• Current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient’s/family’s physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other IDG members in providing daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient’s condition.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
