Job summary

Heart to Heart Hospice seeks a Triage Registered Nurse to provide after-hours telephonic triage for hospice patients and families. The role requires an active RN license, a valid driver’s license, and 2 years of RN experience in a healthcare setting. The position is full-time with comprehensive benefits.

Responsibilities include scheduling on-call visits, documenting care plans, and coordinating referrals while ensuring compliance with Medicare and policy guidelines.

Qualifications

• Current State Registered Nurse license.
• Current State Driver’s License with reliable transportation and auto liability insurance.
• Two years of RN experience, preferably in healthcare/clinical setting.

Responsibilities

• Provide telephonic triage to hospice patients and families during after-hours, weekends, or when office is closed.
• Ensure after-hour visits are scheduled with the on-call team in a timely manner.
• Ensure documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the weekday team.
• Manage referrals that come in over the weekend and after-hours.
• Provide education and support to patients and families/caregivers.
• Conduct patient visits as needed (admissions, routine, pronouncements, tuck-ins).
• Promote positive, respectful communication to all staff, referral sources, patients and families.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
