Job summary

Enhabit Home Health & Hospice is seeking a licensed registered nurse in Alabama to join our nationwide home-based care team. You will coordinate care, assess patient and family needs, and implement the interdisciplinary plan of care in accordance with physician orders and the established plan of care, ensuring compliance with federal and state regulations.

Requirements include graduation from an approved school of nursing, an active Alabama license, and at least one year of RN experience.

Qualifications

• Must graduate from an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year of experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
