Job summary

Enhabit Home Health & Hospice in Montgomery, AL is seeking a licensed RN to join our home-based care team. You will coordinate patient care, assess needs, and implement the interdisciplinary plan of care under a physician's orders.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.
