Pathways Hospice is seeking a Clinical Supervisor RN to plan, coordinate, and deliver high-quality interdisciplinary hospice care. You will ensure compliance with federal/state standards and clinical best practices, while supervising care teams and optimizing patient outcomes.

The role emphasizes mentorship, audit of documentation, and ongoing improvement of care processes in a community-based hospice setting. Texas RN license and hospice experience are preferred.

Qualifications

• 2+ years experience as Hospice RN Supervisor
• Hospice supervisory experience
• Reliable commute to the Branch
• RN licensed in the State of Texas
• Experience with HCHB EMR (preferred)
• Knowledge of Home Health regulations and compliance
• Proficient in Word, Excel, PowerPoint, Outlook

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned hospice care team(s)
• Manages ongoing case management and ensures accurate evaluation and treatment planning
• Reviews electronic documentation and provides feedback to clinicians for compliance and medical necessity
• Reviews agency data and facilitates quality improvement activities
• Reviews patient schedules to ensure coordinated care and efficient workload
• Educates and mentors the home health team on documentation and standards of care
