Job summary

Family Home Health Network in the Chicagoland area is seeking a dedicated Hospice Nurse committed to high-quality patient care. You will perform detailed initial assessments of patients, families, and home environments, monitor nursing needs, and initiate or revise the plan of care as conditions change. Spanish speaking preferred.

Eligible employees may receive a company car and gas card after 90 days. Benefits include health and dental insurance, paid orientation, and paid time off.

Qualifications

• Must possess current Illinois RN licensure.
• Minimum of two years hospice experience preferred.
• Current CPR.
• Valid IL driver’s license and auto insurance.
• HCHB EMR experience a plus.
• Must possess excellent clinical nursing and customer service skills.

Responsibilities

• Perform detailed initial assessments of the patient, family, and home/environment to determine care needs.
• Monitor and regularly re-evaluate nursing needs and initiate/ revise the plan of care as patient status changes.
• Provide high-quality patient and family education and support.
• Communicate effectively with the care team to coordinate services.

Job description

Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

Spanish Speaking Preferred

Qualifications:

• Must possess current Illinois RN licensure
• Possess a minimum of two (2) years experience (Hospice Preferred)
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
