Job summary

Hartford HealthCare at Home is seeking a dedicated Registered Nurse for Hospice Visiting Nurse (RN) roles. You will support patients and families through the end-of-life journey, conduct comprehensive assessments, and collaborate with an interdisciplinary team to manage symptoms and goals for care.

The role requires a CT RN license, at least 1 year of RN experience (homecare or hospice preferred), and a valid driver's license.

Qualifications

• CT RN license is required.
• 1 year RN experience; homecare or hospice preferred.
• Valid driver's license and reliable transportation may be required.

Responsibilities

• Support patients, families, and loved ones during end-of-life journey.
