Hartford HealthCare at Home is seeking a Hospice Visiting Nurse (RN) to support patients and families at end of life in home settings. You will perform comprehensive assessments, set goals for symptom management, and collaborate with an active interdisciplinary team to deliver holistic care.

With a CT RN license, strong communication, and reliable transportation, you will document services using state-of-the-art technology and contribute to our person-centered mission across Connecticut.

Qualifications

• Registered Nurse with CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Support our patients, families, and loved ones, as they go through their end of life journey.
• Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies.
• Be a member of an active interdisciplinary team that addresses patients needs holistically.
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
