Hartford HealthCare at Home is seeking a compassionate Registered Nurse (CT RN license) to join our Hospice Visiting Nurse team in Connecticut. You will conduct comprehensive assessments, set goals for symptom management, and collaborate with an interdisciplinary team to deliver patient-centered end-of-life care at home.

The role requires at least one year of RN experience (homecare or hospice preferred), a valid driver's license, and computer proficiency.

Qualifications

• Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Support patients and families through the end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and care.
• Be part of an interdisciplinary team addressing patient needs holistically.
• Develop and maintain therapeutic relationships with patients and families.
• Document skilled services using state-of-the-art technology.
