Absolute Hospice, Inc. is seeking a Registered Nurse for PRN and weekend on‑call hours to provide compassionate hospice care in patients' homes. You will establish professional relationships with patients, families, and healthcare teams, delivering hands-on care and clinical management in a home setting.

You will educate patients and caregivers on conditions and care steps, perform initial evaluations, and participate in ongoing QA/QI processes to ensure high-quality care.

Formación

• Must hold an Associate's of Science degree in Nursing.
• Valid Ohio RN license.
• Minimum 2 years of nursing experience including hospice or home health preferred.
• Valid driver's license and one year clean driving record.
• Excellent writing, communication and charting skills.

Responsabilidades

• Making initial nursing evaluation visit
• Continually evaluating the patient's needs
• Initiating, coordinating and revising the treatment plan
• Rendering nursing care and performing treatments, as ordered by the physician
• Participating in the Quality Assurance/Quality Improvement process

The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands‑on‑care, on‑going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

Additional responsibilities of our Hospice RN include:

Requirements

• Caring, patient and flexible teacher with excellent communication and time management abilities
• Possess independent problem solving abilities, excellent clinical assessment skills
• Computer literate
• Good physical shape
• Ability to lift/transfer patients to and from wheelchairs, if necessary
• Associate's of Science degree in Nursing
• Valid Ohio RN license
• Minimum 2 years of nursing experience including hospice or home health experience preferred
• Valid driver's license and one year clean driving record
