High Plains Senior Care Hospice is seeking an RN to provide in-home nursing care for clients in Eden, TX, PRN. You will coordinate with the interdisciplinary group, families and referring agencies to ensure a comprehensive Plan of Care and timely documentation.

The role requires an RN license in TX (or NCL), 2 years of RN experience in clinical settings, and strong communication and interpersonal skills. CPR, TB screen and Hep B vaccination are expected per policy.

Formación

• Texas RN license or Nurse Licensure Compact (NCL) eligibility.
• Two years' RN experience in clinical care; hospice/home health preferred.
• Strong nursing skills, communication, and interpersonal abilities.

Responsabilidades

• Promote hospice mission, goals and philosophy.
• Coordinate care with IDG, patient/family and referring agency.
• Provide nursing care per Plan of Care and document services.
• Perform information visits, eligibility assessments and admissions.
• Attend IDG meetings and participate in ongoing education.
• On-call duties as assigned.
