Job summary

Care Initiatives Hospice is seeking an RN Hospice Visit Nurse to provide compassionate, high-quality care across Council Bluffs, Avoca, Dunlap, and surrounding communities. This PRN role offers in-home and facility-based patient care with a focus on comfort at the end of life.

The RN must have at least two years of recent clinical nursing experience, hold an active RN license, and maintain CPR certification.

Qualifications

• RN with minimum 2 years of recent clinical nursing experience required.
• Current CPR certification required.
• Demonstrated patience and supportive approach to patient, family, and caregiver support.

Responsibilities

• Collaborate with the physician to oversee and implement personalized care plans with interdisciplinary teams.
• Provide high-quality patient care in home and facility settings and ensure accurate clinical documentation.
• Perform initial and comprehensive assessments in accordance with policies.
• Update care plans as needed with patient and family input.

What it takes to become a Hospice Visit Nurse:
