Job summary

Care Initiatives Hospice is seeking an RN Case Manager who provides compassionate, patient-centered care in home, long-term care, assisted living, or hospital settings. You will assess, plan, and coordinate care with physicians, families, and an interdisciplinary team, ensuring comfort and dignity for patients at the end of life.

The role requires an Iowa RN license, CPR certification, hospice experience, and the ability to travel within your region.

Qualifications

• Active Iowa RN license is required.
• CPR certification is required.
• Hospice or palliative care experience preferred.
• Two years RN/LPN experience preferred.
• Must have a valid driver’s license and insurance; travel within region.
• Home internet access.
• Ability to obtain CHPN within 24 months of employment.

Responsibilities

• Use clinical nursing knowledge to deliver high quality patient care in patient homes or care settings.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies.
• Conduct ongoing visits and interdisciplinary assessments.
• Provide clear, concise, accurate clinical documentation.
