Caris Healthcare is seeking a Registered Nurse (RN PRN) to provide compassionate hospice/home health care per physician orders. The role emphasizes assessment, planning, implementing, and evaluating total patient care and nursing care plans for each patient.

Qualifications

• Must be a licensed RN in the operation state.
• Hospice or Home Health experience preferred.
• Solid knowledge of nursing principles and patient-family teaching.

Responsibilities

• Assess, monitor, and document patient symptoms using required software.
• Collaborate with patient, family, and interdisciplinary team to develop individualized care plans.
• Serve as patient advocate and explain rights, reimbursement procedures, and responsibilities.
• Observe and evaluate physical and emotional status, safety factors, and caregiver education needs.
• Supervise and direct aides and licensed practical nurses per guidelines.
• Participate in on-call rotations and provide back-up as requested.
