Caris Healthcare is seeking a registered nurse to provide hospice nursing care to patients in Columbia, SC, following physician orders and established care plans. You will assess, implement, and evaluate total patient care while coordinating with families and the interdisciplinary team.

The ideal candidate has hospice or home health experience, strong nursing knowledge, and a commitment to compassionate, patient-centered care.

Qualifications

• Must be a licensed RN in the operating state.
• Nursing experience required, hospice or home health preferred.
• Strong knowledge of nursing principles and patient/family teaching.

Responsibilities

• Assess, monitor, and document patient symptoms using required software.
• Collaborate with patient, family, partners, and the care team to plan care.
• Advise on patient rights, grievance procedures, and reimbursement processes.
• Evaluate patient status and education needs for caregivers.
• Supervise and direct aides and licensed practical nurses per regulations.
• Participate in on-call rotations and provide back-up support as needed.
