Qualifications
- Must be a Registered Nurse licensed in the state of operation.
- Nursing experience required, preferably in Hospice or Home Health.
- Sound knowledge of nursing principles and patient family teaching.
Responsibilities
- Assess, monitor, and document patient’s symptoms using required software.
- Collaborate with patient, family, and interdisciplinary team on care plans.
- Serve as patient advocate, providing information on rights and responsibilities.
- Observe and evaluate patient’s status and family dynamics.
- Supervise aides and licensed practical nurses.
- Participate in on-call rotating schedule.
