Responsibilities include:

• Assuming full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
• Making the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifying the patient's/family's physical, psychosocial, and environmental needs, and re-assessing as needed, at least every 15 days.
• Initiating and coordinating the plan of care.
• Documenting problems, goals, interventions, and patient/family responses.
• Collaborating with the patient/family, attending physician, and other IDG members for daily care.
• Instructing and supervising the patient/family in self-care techniques when appropriate.
• Maintaining accurate clinical notes regarding the patient's condition.

Experience Requirements

• One year of clinical experience.

License Requirements

• Current RN licensure in the state of practice.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, access to a dependable vehicle or public transportation.

Additional State Specific Requirements

• LA: Minimum two years of full-time RN experience; hospice care experience as a licensed practical nurse may substitute for RN experience.
