Trinity Health at Home is seeking an experienced PRN RN remote Clinical Quality Review Specialist with ICD10 HCS-D certification, which is required.
OASIS COS-C certification required within 6 months of hire.

Position Overview

The Clinical Quality Review Specialist is a remote position responsible for processing and completing assigned quality and clinical workflows, identifying errors and omissions in clinical assessments, OASIS, coding, plans of care, and other clinical documentation. The role involves collaborating with clinicians to ensure accurate documentation and development of plans of care, including visit utilization targets and hospitalization risk identification. The specialist provides education to clinicians on quality, outcomes improvement, documentation standards, and clinical process enhancements. Participation in department quality improvement initiatives and collaboration on organization-wide initiatives are also key responsibilities.

Minimum Qualifications

• Registered Nurse with a current license from the practicing state; additional state licenses may be required.
• Degree: Bachelor's preferred; associate degree considered if complemented by demonstrated career progression and performance.
• ICD10 HCS-D certification is required.
• OASIS COS-C certification is strongly preferred.
• Experience with Home Care Home Base software is a plus.
• Experience with EPIC software is a plus.
• Strong computer and technology skills are required.
• Excellent interpersonal and communication skills for effective interaction with diverse stakeholders.
