Hartford HealthCare at Home is seeking a Hospice Visiting Nurse RN to support patients and families at end of life in Connecticut. The role requires an active CT RN license and at least one year of RN experience, preferably in homecare or hospice settings.

A valid driver's license and computer proficiency are also required. You will work within an interdisciplinary team, perform assessments, set goals for symptom management, and document services using modern technology.

Qualifikationen

• Must be a Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.
