Job summary
Hartford HealthCare at Home is seeking a Hospice Visiting Nurse to support patients, families, and loved ones through the end-of-life journey. This role emphasizes patient-centered care, symptom management, and collaboration with an interdisciplinary team.
Graduates are welcomed, but an RN with CT license and at least one year of RN experience is required; a valid driver's license and reliable transportation are essential; computer proficiency and EMR experience are preferred.
Qualifications
- Registered Nurse with Valid CT RN license.
- One year of RN experience; Homecare or Hospice experience preferred.
- Valid driver's license and reliable transportation required.
- Computer proficiency required.
Responsibilities
- Support our patients, families, and loved ones, as they go through their end of life journey
- Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies
- Be a member of an active interdisciplinary team that addresses patients needs holistically
- Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
- Utilize state-of-the-art technology to document skilled services provided.
