Job summary

Bristol Hospice Phoenix, AZ is seeking a compassionate Registered Nurse (RN) for PRN hourly work to assess and care for patients at home or in facilities within the Bristol Hospice service area. You will collaborate with the interdisciplinary team to plan and deliver high-quality hospice care with dignity and respect.

The role requires at least one year in acute care, current state licensure, willingness to travel, and a commitment to compassionate end-of-life care within a supportive team

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of home care experience is preferred.
• Current RN licensure in the state.
• Willingness to travel throughout Bristol Hospice service area.

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide comprehensive physical assessment and history of illnesses.
• Deliver professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Regularly re-evaluate patient and family needs and adjust care plans.
• Collaborate with physicians and interdisciplinary team.
• Educate patient and family on self-care techniques.

On an An Average Day you Will:

• Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs
• Provide a complete physical assessment and history of current and previous illness(es)
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient’s status by:
• Writing and initiating plan of care
• Regularly re-evaluating patient and family/caregiver needs
• Participating in revising the plan of care, as necessary
• Initiate the plan of care and make necessary revisions as patient status and needs change
• Use health assessment data to determine nursing diagnosis
• Develop a care plan that establishes goals, based on nursing diagnosis
• Include the patient and the family in the planning process
• Record pain/symptom management and changes/outcomes as appropriate
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders, as required
• Teach the patient and family/caregiver self-care techniques, as appropriate
• Work in concert with the interdisciplinary group
• All other duties as assigned

Requirements:

• Must have at least one (1) year of acute care experience in an institutional setting
• One (1) year of experience in home care preferred
• Must have a current licensure in the state
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate nursing skills per competency checklist
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
• Ability to travel throughout the Bristol Hospice Service Area
