Job summary

Bristol Hospice is seeking a compassionate Registered Nurse (RN) for our Northern Utah service area (Ogden). The PRN/relief position requires ICU or acute care experience, current Utah license, and willingness to travel within Tremonton to Bountiful coverage.

Qualifications

• Minimum one year acute care experience in an institutional setting.
• One year home care experience preferred.
• Current Utah nursing license required.
• Hospice philosophy understanding and ability to care for terminally ill patients.
• Willingness to travel within Bristol Hospice service area.

Responsibilities

• Conduct comprehensive patient and family assessments to determine hospice needs.
• Provide thorough physical assessments and histories of illnesses.
• Deliver professional nursing care using the nursing process.
• Develop and revise care plans with patient and family input.
• Document pain and symptom management and outcomes.
• Communicate with physicians about patient needs and orders.
• Educate patients and families on self-care techniques.
• Collaborate with the interdisciplinary team to meet goals.
• Perform other duties as assigned.

Qualifications

On an An Average Day you Will:

• (Includes, but not limited to)


• Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs


• Provide a complete physical assessment and history of current and previous illness(es)


• Provide professional nursing care by utilizing all elements of the nursing process


• Assess and evaluate patient’s status by:


• Writing and initiating plan of care


• Regularly re-evaluating patient and family/caregiver needs


• Participating in revising the plan of care, as necessary


• Initiate the plan of care and make necessary revisions as patient status and needs change


• Use health assessment data to determine nursing diagnosis


• Develop a care plan that establishes goals, based on nursing diagnosis


• Include the patient and the family in the planning process


• Record pain/symptom management and changes/outcomes as appropriate


• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders, as required


• Teach the patient and family/caregiver self-care techniques, as appropriate


• Work in concert with the interdisciplinary group


• All other duties as assigned

Requirements

• Must have at least one (1) year of acute care experience in an institutional setting


• One (1) year of experience in home care preferred


• Must have a current licensure in the state


• Must understand the philosophy of hospice concept and understand needs of the terminally ill


• Must be flexible in work hours


• Must demonstrate nursing skills per competency checklist


• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs


• Ability to travel throughout the Bristol Hospice Service Area
