Commonheart is seeking a dedicated Hospice Patient Care Coordinator (PCC) in Wisconsin to provide clinical leadership and mentoring for staff. The PCC ensures quality, patient-centered care and promotes the organization’s mission and values throughout the community.

Responsibilities include supervising nursing staff, supporting patient visits, collaborating with the IDT, and guiding care plans while maintaining strong community relations and adherence to standards.

Qualifications

• Current State of Wisconsin RN license.
• Four to five years of Hospice experience; four year degree preferred.
• Valid WI Driver's license.

Responsibilities

• Maintain and promote effective communication with residents and families.
• Supervise and direct care of RNs, LPNs, and Hospice Aids.
• Triage patient care visits and support RN Case Manager responsibilities as needed.
• Lead interdisciplinary team meetings to plan and review patient care.
