Commonheart seeks a Hospice Patient Care Coordinator (PCC) to provide clinical leadership and mentoring for Hospice staff in Wisconsin. You will oversee care plans, support interdisciplinary teams, and drive adherence to our mission and quality standards.

Responsibilities include supervising RNs/LPNs and Aides, guiding care visits, coordinating with intake, and ensuring policy compliance while maintaining excellent resident and family communication.

Qualifications

• WI RN license required.
• Four to five years hospice experience.
• Four year degree preferred.
• Valid WI Driver's license.

About the Role

The Patient Care Coordinator (PCC) is responsible to provide clinical leadership, guidance, and mentoring support for Hospice and its staff.

Essential Job Functions

1. Maintains and actively promotes effective communication with residents and family members/personal representatives.
2. Responsible for maintaining a positive image of Commonheart in the community keeping in alignment with our mission, vision and values.
3. Supervise and direct care of other nursing personnel including RNs, LPNs and Hospice Aids.
4. Support/triage patient care visits and clinical questions and serve as RN Case Manager, as needed.
5. Teach appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and other as appropriate.
6. Lead, collaborate, and participate in interdisciplinary team (IDT) meetings to establish, review and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
7. In conjunction with Director, lead assigned Hospice region to achieve the organizational annual goals and objectives and assures implementation as scheduled.
8. Support the hospice intake and referral process to ensure timely and appropriate response to inquiries and admissions.
9. Work with providers and the hospice team to gather/review the patient specific medical information to ensure appropriate patient eligibility.
10. Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested.
11. Responsible for ensuring that all corporate policies and procedures are administered effectively and handled in a fair manner.
12. Responsible for fostering collaborative relationships with area partners and for managing the admissions processes.
13. Provide back up and support to other Hospice regions and/or PCCs as needed.
14. Responsible for compliance with all local, state, and federal codes pertaining to the operations of the agency.
15. Responsible for on-call services as assigned.
16. Performs other similar duties, as needed.

Requirements

1. Commitment to quality programs and services for the residents.

3. Ability to relate well to the residents/ families, staff, churches and the community in general.
4. Ability to maintain and protect the confidentiality of information.
5. Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.

Experience & Training

1. Current State of Wisconsin (WI) Registered Nurse license.
2. Four to five years of Hospice experience. Four year degree preferred.
3. Valid WI Driver's license.

Hours

• Monday - Friday 8:00 AM to 4:30 PM
• Rotating On-Call - Every 6th weekend

Pay

• $88,400.00 to $105,352.00 / yearly

(Higher pay may be considered depending on qualifications and experience)
