Job summary

Legacy Hospice, Inc. is seeking a Patient Care Coordinator (PCC) in Cape Girardeau, MO.

This full-time, salaried, exempt role leads the clinical team, oversees day-to-day patient care coordination, and supports operations to ensure regulatory compliance and high-quality hospice services. The PCC will supervise staff, participate in IDT meetings, manage admissions/discharges, and provide RN case management as needed.

Qualifications

• RN licensed in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of hospice, home health, or related nursing experience.
• Experience supervising clinical and paraprofessional staff.
• Excellent leadership, communication, and time management skills.

Responsibilities

• Oversee day-to-day coordination of patient care.
• Supervise the interdisciplinary clinical team.
• Support Branch Administrator in operations and compliance.
• Lead IDT meetings and participate in staff hiring and evaluations.
• Oversee admissions, discharges, transfers, and regulatory reporting.
• Provide RN case management and direct patient care as needed.
• Collaborate with providers and internal departments.
• Act as Branch Administrator when needed.
