Job summary

Caris Healthcare in Sumter, SC is seeking a Registered Nurse (RN) to join our hospice and home health team. You will assess patients, develop care plans, implement nursing interventions and document outcomes in the required software, delivering compassionate, patient-centered care per physician orders.

Responsibilities include collaborating with patients, families and the interdisciplinary team; acting as patient advocate; supervising aides; and participating in on-call rotation.

Qualifications

• Active RN license in the state of operation.
• Hospice or Home Health experience preferred.
• Strong knowledge of nursing principles and patient/family teaching.

Responsibilities

• Assess, monitor, and document patient symptoms per required software.
• Collaborate with patient, family, and interdisciplinary team to develop individualized care plans.
• Advocate for patients and inform on patient rights and responsibilities.
• Observe and evaluate patient status to determine education needs for caregivers.
• Supervise and direct aides and LPNs; participate in on-call rotation.
